- I respectfully request a formal written letter of reference or professional recommendation from my direct supervisor or an appropriate member of management, to be provided prior to or shortly after my last day of employment.
- I request full payout of all accrued and unused Paid Time Off (PTO), vacation days, and any other compensable leave balances in accordance with applicable state wage-payment law and company policy, including (as applicable) Cal. Labor Code §§ 201-203 (with waiting-time penalties for late payment of final wages), N.Y. Labor Law § 191, and Tex. Labor Code § 61.014. I understand this payout will be included with or issued separately from my final paycheck.
- Equipment to Return: MacBook Pro 16" (Asset #MK-4821), company iPhone 15 (Asset #PH-1190), building access badge, parking garage remote. I will ensure all company property is returned in good condition on or before my last day of employment.
