1. GRANT OF AUTHORIZATION

I, Robert A. Martinez, a legal adult residing at the address stated above, do hereby authorize Laura M. Martinez (my Spouse) to collect, pick up, receive, and/or sign for documents on my behalf. This grant of authority is established under the common law of agency (see Restatement (Third) of Agency §§ 1.01, 2.01–2.02, 3.01), under which the Authorizer (Principal) authorizes the Authorized Person (Agent) to act on the Principal's behalf within the scope defined herein. This authorization is granted voluntarily and of my own free will. The Authorized Person shall present this letter along with a valid government-issued photo identification when exercising the authority granted herein.

2. SCOPE OF AUTHORIZATION

The Authorized Person is specifically empowered to perform the following tasks and actions on my behalf:

Pick up certified copies of birth certificates for our minor children from the Sangamon County Clerk's Office.
Sign any required forms or receipts for document release.
Pay any applicable fees using personal funds or check.

The Authorized Person shall act in good faith and in my best interest at all times. Any actions taken outside the scope of this authorization shall be considered unauthorized and shall not be binding upon the Authorizer.
3. DURATION AND VALIDITY

This authorization shall become effective on March 25, 2026 and shall remain in full force and effect until April 25, 2026, unless revoked earlier in writing by the Authorizer. Upon expiration or revocation of this authorization, the Authorized Person shall immediately cease all activities authorized herein and shall return any documents, keys, or other items received pursuant to this authorization.
4. LIMITATIONS AND RESTRICTIONS

The following limitations and restrictions apply to this authorization. The Authorized Person shall not exceed or act outside these boundaries:

This authorization is limited to the Sangamon County Clerk's Office only. The authorized person may not request or obtain any documents other than the birth certificates specified above.
5. REVOCATION

I reserve the right to revoke this authorization at any time by providing written notice to the Authorized Person and to any relevant third parties who have been presented with this letter. Revocation shall be effective upon receipt of such written notice. Any actions taken by the Authorized Person prior to the effective date of revocation shall remain valid and binding, provided they were within the scope of this authorization.
6. LIABILITY AND INDEMNIFICATION

The Authorized Person agrees to exercise due care and diligence in performing the authorized tasks. The Authorizer shall not be liable for any unauthorized actions taken by the Authorized Person that fall outside the scope of this letter. The Authorized Person agrees to indemnify and hold harmless the Authorizer against any claims, damages, losses, or expenses arising from the Authorized Person's negligence, willful misconduct, or unauthorized actions taken under the purported authority of this letter.
7. THIRD-PARTY RELIANCE

Any person, institution, company, or government entity dealing with the Authorized Person within the scope and time period of this authorization may rely upon the authority granted herein. I agree to ratify and confirm any acts properly performed by the Authorized Person in accordance with the terms of this letter. A photocopy or electronic copy of this authorization letter shall have the same force and effect as the original for purposes of third-party reliance.
8. GOVERNING LAW

This Authorization Letter shall be governed by and construed in accordance with the laws of the state in which the Authorizer resides. Any disputes arising out of or relating to this letter shall be resolved in the courts of competent jurisdiction in the Authorizer's state of residence.
9. DISCLAIMER

This Authorization Letter does not constitute a durable power of attorney under the Uniform Power of Attorney Act (UPOAA) §§ 101–123, an IRS tax power of attorney (Form 2848), a DHS representation entry (Form G-28), or a HIPAA-compliant authorization (45 C.F.R. § 164.508) unless the separate statutory requirements of those frameworks are independently satisfied. It does not grant the Authorized Person legal authority to enter into contracts binding the Authorizer, make substantive financial or healthcare decisions, or take legal actions beyond the specific scope described herein. For broader legal authority, a properly executed statutory power of attorney should be obtained.
