I hereby authorize my employer to initiate credit entries and, if necessary, debit entries and adjustments for any credit entries made in error, to the bank account(s) indicated above in accordance with the Automated Clearing House (ACH) rules and regulations established by the National Automated Clearing House Association (NACHA Operating Rules).

This authorization is voluntary and is given with knowledge of its terms. It shall remain in full force and effect until I notify my employer in writing that I wish to revoke it. I understand that my employer requires reasonable notice (typically one full pay cycle) to process any changes or cancellations to this direct deposit arrangement, and that payment of wages remains subject to the timing requirements of the Fair Labor Standards Act, 29 U.S.C. § 206, and applicable state wage-payment laws.

I acknowledge and agree to the following terms:

1. It is my responsibility to verify that all account information provided above is accurate and complete. I understand that incorrect information may result in delayed or misdirected payments, and my employer shall not be held liable for deposits made to incorrect accounts based on information I have provided.

2. This authorization applies to all payroll deposits including regular wages, bonuses, commissions, expense reimbursements, and any other compensation owed to me by my employer, unless otherwise specified in writing.

3. In the event that a deposit is made in error (whether in the wrong amount or to the wrong account), I authorize my employer and the financial institution to make the necessary corrective debit entries to recover the funds, consistent with the NACHA Operating Rules governing reversals and the consumer-protection limits on unauthorized-transfer liability under the Electronic Fund Transfer Act, 15 U.S.C. §§ 1693–1693r, and Regulation E, 12 C.F.R. Part 1005.

4. I will promptly notify my employer in writing of any changes to my banking information, including account closures, new accounts, or changes to routing numbers.

5. Direct deposit may take one to two pay cycles to become effective after initial enrollment or changes. During this interim period, my employer may issue paper checks.

6. I understand that my employer complies with all applicable federal and state laws governing electronic fund transfers, including the Electronic Fund Transfer Act (EFTA), 15 U.S.C. §§ 1693–1693r, and its implementing Regulation E at 12 C.F.R. Part 1005, as well as state wage-payment statutes that condition payroll direct deposit on employee choice and written authorization (including, for example, Cal. Labor Code § 213 and N.Y. Labor Law § 192). I have not been required to accept direct deposit as a condition of employment where state law prohibits that condition, and I may revoke this authorization at any time by written notice.
