1. GRANT OF CONSENT. I/We, the parent(s) or legal guardian(s) of Olivia Rose Thompson, born 12 June 2018, holder of Australian passport number PA1234567, hereby consent to Olivia Rose Thompson travelling internationally to Tokyo and Osaka, Japan in the company of Jessica A. Thompson (Mother) from 1 June 2026 to 15 June 2026. Travel will be via Qantas Airways, Flight QF 025.
2. PARENTAL RESPONSIBILITY. Under s. 61C of the Family Law Act 1975 (Cth), each parent of a child has parental responsibility for the child. Under s. 65Y of the Act, taking a child from Australia without the consent of all persons with parental responsibility is a criminal offence. I, Michael R. Thompson, as a parent with parental responsibility for the above-named child, hereby give my consent to this international travel.
3. MEDICAL INFORMATION. The following medical information is provided for treating health professionals:

4. MEDICAL AUTHORISATION. In the event of a medical emergency during travel, the travelling parent or guardian is authorised to consent to any necessary medical treatment, including surgery and hospitalisation, for the child when the non-travelling parent or guardian cannot be contacted in a timely manner. This authorisation extends to any licensed medical practitioner, dentist, hospital, or medical facility in Australia or overseas.
5. AUSTRALIAN BORDER FORCE REQUIREMENTS. The consenting parent(s)/guardian(s) acknowledge that:
• All children under 18 years travelling internationally from Australia may be required to provide parental consent documentation to the Australian Border Force (ABF) or a foreign border authority.
• A valid Australian passport is held by the child (No. PA1234567).
• A copy of the child's birth certificate or Australian citizenship documentation may be required by foreign immigration authorities.
• The destination country may impose its own entry requirements for minors.
6. SCOPE AND DURATION. This consent is valid solely for the travel described in this document and expires on 15 June 2026. This document does not transfer legal custody or guardianship. The travelling parent or guardian must carry this consent form during travel and present it to border and immigration authorities upon request. A copy of this form has the same effect as the original.

Having this consent letter witnessed and/or notarised by a Justice of the Peace (JP), notary public, or Australian legal practitioner is strongly recommended for international travel. Some destination countries require a notarised or apostilled copy.

What Is a Child Travel Consent Form?
A child travel consent form is a written authority from the non-travelling parent or guardian permitting a child under 18 to travel with a specified adult. It identifies the travelling adult, the destination, the dates of travel, and confirms that the other parent or guardian consents. It is commonly requested by airlines, immigration officers, and overseas authorities to prevent international child abduction.
Under the Family Law Act 1975 (Cth), both parents generally have parental responsibility. Where a parenting order is in place under Part VII, removing a child from Australia without the written consent of all persons in whose favour the order was made is a criminal offence under section 65Y and section 65Z, punishable by up to three years' imprisonment. A travel consent form evidences the required authorisation.
Australia is a party to the Hague Convention on the Civil Aspects of International Child Abduction 1980, implemented by the Family Law (Child Abduction Convention) Regulations 1986 (Cth). A travel consent form does not override the Convention's return mechanism but clearly evidences lawful travel, reducing the risk of the child being detained at borders or being the subject of a Hague return application.

Sections 65Y and 65Z of the Family Law Act
Where a parenting order is in place, sections 65Y and 65Z of the Family Law Act 1975 (Cth) make it a criminal offence to take a child outside Australia without the written consent of each person in whose favour the order was made or without a court order permitting the removal. Penalty: up to three years' imprisonment. A written travel consent form, signed by the non-travelling parent, is critical evidence of compliance.
Hague Convention on International Child Abduction
Australia is a contracting state to the Hague Convention on the Civil Aspects of International Child Abduction 1980, implemented domestically by the Family Law (Child Abduction Convention) Regulations 1986 (Cth). Wrongful removal or retention of a child in breach of custody rights can lead to an application for the child's return to Australia through Convention central authorities.
Australian Border Force Requirements
The Australian Border Force may question travellers with children where parental consent appears questionable. While a notarised consent form is not strictly required by Australian law, many destination countries — including South Africa, Mexico, and various European jurisdictions — require notarised or apostilled consent. Passport Controls
Under the Australian Passports Act 2005 (Cth), a child's passport application generally requires the consent of all persons with parental responsibility. Where a passport has been issued, section 22 allows the Minister to refuse or cancel a passport at the request of a person with parental responsibility to prevent international abduction.
